STEP 1: Download/Print "Registration Form.pdf" attachment from below. FIll it out in with correct info and sign WAIVER (one form per student).

A. 3 Posted Cheques DATED   01.SEP.10 - 01.DEC.10 - 01.MAR.11

B. 3 Visa/Mastercard Transactions ( 1.SEP-NOV 2. DEC-FEB 3. MAR-MAY)

C. Automatic Withdrawl  - MONTHLY OPTION ONLY (depit-void cheque required).

D. Payment in FULL - Cash/Credit Card/Cheque - ( REGISTRATION FEE WAIVED & 5% DISCOUNT).

Remember all credit card transactions are subject to a 4% service/admin charge.

STEP 3. Downlaod/Print Fee's/Policies form READ CAREFULLY and sign.


STEP 4.  Mail ALL forms to studio address with non-refundable registration fee $30 (made out to Boston Dance Collective or BDC) dated the day you register and form of payment.

**Make sure to send in a seperate cheque for costumes made out to BDS. $50 per class.**

Once the studio has receievd ALL nessary information and payment a confirmation may be sent to you via email. Thank you

You are only registered once office administration recieves the following:

1.* Full Signature on our payment & policies sheet/registration form, which you must download below ("Payment Policies Form.pdf")

2.* Registration Fee $30.00 Per student $15.00 for any additional family members (made out to BDC.)

3.* FULL payment plan either Automatic Withdrawl/Credit Card or 3 Postdated Cheques.

4.* All costume Deposits of $50.00/per class made out to Boston Dance Society (BDS)
